If a participant is discharged after completing three or fewer visits, complete an Abbreviated Discharge Summary and leave the remaining sections of the full report blank. This summary should include, but is not limited to, information about the participant’s reentry needs, attendance, level of engagement, any progress made, and a clear explanation of the circumstances that resulted in the early discharge.
